| Create/Change/Correct Address for New or Existing Property
Information and Requirements for Requests for New Addresses
If you need to create a new address, change an existing an address, or correcting an error in their current address in the District of Columbia, you need to complete the Address Change Request Application*.
Along with the application, you need to submit the following:

A detailed site plan identifying the location(s) of the specified lot(s) and building(s). You need to identify the street frontage of the main entrance of the building(s) for which the address creation, change or corrections is being requested. On the site plan, please indicate the two adjacent addresses, above and below the subject lot(s) if available, and all pertinent cross streets.

You are exempt from submitting a site plan if you are seeking multiple addresses for a two family flat, however, a copy of a valid Certificate of Occupancy must accompany the application form. Separate addresses for two family flats will be identified by a suite number or an A or B.

Independent street numbers are reserved for buildings fronting on multiple streets with separate uses or with separate lot numbers with the exception of properties designated as condominiums.
NOTE: Please use a separate sheet of paper to identify individual lots and corresponding requested addresses, if any, if your request is for multiple addresses.

No official address(es) shall be assigned to any public street(s), the name of which has not been approved by the D.C City Council. See the District of Columbia Code 1995 replacement Volume 4A, Chapter 7, Section 7 - 455.

When a principal building has no street frontage, all zoning requirements as per the District of Columbia Municipal Regulations Title 11 “Zoning” Article 25, Section 2516.5 shall be met and approved before an address can be assigned.
