Application Requirements

All applicants for a Basic Business License must comply with the following DC Code requirements:

Basic Business License Application
All applicants for a Basic Business License must complete an application, remit the associated payment, and submit the required supporting documentation.

Clean Hands Self-Certification
All applicants must certify that no more than $100 is owed to the District of Columbia Government as a result of fees, penalties, interest, or taxes through completion of a Clean Hands Self-Certification. 

Tax Number
All Applicants must provide a registered tax number (i.e., SSN or FEIN) at the time of application. Register by completing and filing a Combined Business Tax Registration Application (Form FR-500) with the Office of Tax and Revenue, 1101 4th Street SW, Suite W270, Washington, DC 20024. Online registration is available at the Business Tax Service Center. For more information, please call the Tax Customer Service Center at (202) 727-4829.

Certificate of Occupancy
All applicants for a Basic Business License operating from a premise located in the District of Columbia must provide a Certificate of Occupancy (C of O) for the premise address from which the business activity is conducted in order to demonstrate the activity does not conflict with building and zoning codes. Note: In some cases, a business operating in an office building may operate under the umbrella of the C of O issued to the owner of the building as long as the C of O is issued for the entire building. Check with the building owner or management company for the holder’s name, C of O number, and issue date.

Police Criminal History Report
All applicants for a Basic Business License for the Parking Facility Attendant category must provide a current Police Criminal History Report. A resident of the District of Columbia may obtain a Police Criminal History Report (Form PD-70) from the Metropolitan Police Department, Metropolitan Police Criminal History Division at 300 Indiana Avenue N.W., Room 3305, Washington, DC 20001, 727-4245. A non-resident must obtain a Police Criminal History Report from the jurisdiction of which he/she is a resident.  The report must no older than 30 days from the time of application. Please provide court documentation/dispositions regarding any criminal conviction(s) to avoid delay in the review of your application.

Affidavit
All applicants for a Basic Business License for the Parking Facility Attendant category must include an Affidavit sworn to before a notary public, by a Parking Facility located in and licensed to engage in the Parking Facility business in the District of Columbia, certifying that the applicant will be employed by the parking establishment.

Digital Passport Photographs
All applicants for a Basic Business License for the Parking Facility Attendant category must apply in-person and submit three (3) digital passport photos. The Department of Consumer and Regulatory Affairs reserves the right to reject new or renewal applications by mail if they do not meet DC Code and DC Municipal Regulations.

Identification
A copy of the applicant’s driver’s license or non-driver’s identification containing the Applicant’s name, current address, and photograph is required.

Driver’s License
If the applicant will operate a motor vehicle while employed at the parking facility, a copy of the District of Columbia Motor Vehicle Operator’s Permit must be submitted with the application. Add your information in the space set-aside for restrictions:

"The holder of this permit possesses Parking Facility Attendant license no. ______________ expiring _______.”

Non Driving Parking Lot Attendants
If the applicant is a non-driving Parking Facility Attendant, an affidavit from the owner or manager of the parking establishment stating that the applicant will, under no circumstances, be required or allowed to operate a motor vehicle while employed as a Parking Facility Attendant at the parking facility. 

Parking Facility Attendant Designation Letter
All applicants for a Basic Business License for the Parking Facility Attendant category shall include from each licensed Parking Facility for whom the applicant proposes to act as a parking attendant a Parking Lot Attendant Designation Letter stating that the applicant will, if licensed, be employed by that parking facility.

Investigation/Approval
This category of the Basic Business License is subject to a thirty (30) day background investigation prior to issuance of a license. This investigation will be conducted after a Basic Business License Application, all required supporting documentation, and payments have been received and accepted by the Business Licensing Division.

Online.
You may apply for a Basic Business License and pay with a major credit/debit card via the Online Application System (OSUB). Some categories are available for immediate printing. Apply online now!                 

In-Person.
You may apply for a Basic Business License in-person with cash, check, money order or a major credit/debit card by visiting the DCRA, Business License Center, 1100 4th Street, SW, 2nd Floor, Washington, DC 20024. No appointments are required. Applications are processed on a first-come, first-served basis. Note: American Express is not accepted in the Business License Center.

Via Mail.
You may apply for a Basic Business License and pay with a check or money order via mail. Please submit all requirements including the associated fee. Make check or money order payable to “DC Treasurer.”
